What percentage of the cost will CRC subsidize?

The subsidy is 75% of the total cost (monthly fee plus tax) of an individual membership, to a maximum reimbursement of $55 per month.

How do I request a subsidy?

To request reimbursement, log on to My Info and select Main Menu>Employee Self Service>My Wellness>Gym Subsidy Request.

Can I be reimbursed for more than one gym membership?

No. You can be reimbursed for only one membership, even if the total reimbursement would have been less than $55 per month.

If I paid for six yoga sessions but only used four this month, what can I submit for reimbursement?

You can submit a prorated reimbursement request only for the fitness classes you attended. You must have gone to the gym and/or attended fitness classes at least four times in a month to be eligible for reimbursement. You may obtain reimbursement for the remaining two sessions in the month(s) you attend the sessions.

How do I submit a claim for reimbursement for both my monthly fee and the cost of the classes I took that month?

You should combine the cost into one entry for the month. Be sure to retain documentation of the costs.

Is there a deadline date for submitting reimbursement requests?

You must be a CRC employee at the time reimbursement is made, and you must submit your request no later than January 31 following the calendar year you incur the expense.

When can I expect reimbursement?

You can submit expenses at any time, and you will be reimbursed in your normal paycheck within 45 days of submission.

Is the subsidy taxable?

Yes, it will be taxed as ordinary income and taxes will be withheld from your reimbursement.

Do I need to submit and/or retain receipts?

You do not need to submit receipts with your reimbursement request; however, CRC will perform random audits and may request copies of your contract and/or receipts at any time. You should retain, for at least two years, a copy of your contract and proof of payment (e.g., credit card or debit card statements, bank statements, copies of personal checks, etc.), and documentation necessary to show proof of the individual membership rate if you have a family membership (e.g., letter from facility or dated screen print from website).

Does the subsidy apply to family memberships?

If you have a family membership, your reimbursement will be limited to 75% of the individual membership rate up to $55 per month.

Can I submit a claim for my spouse/partner or a child rather than for myself?

No, subsidies are for CRC employee memberships only.

My gym charges a separate initiation fee, is that eligible for the subsidy?

No, the initiation fee is not reimbursable under the subsidy program. Note: CRC has negotiated contracts with several nationwide gyms which have agreed to waive or reduce the initiation fee.

I can get a discounted rate if I pay the full cost of a gym membership for one year in advance. Can I submit the full one-year cost for reimbursement now?

No. You will need to calculate the equivalent monthly rate and indicate the applicable time period on the reimbursement form. You may only submit up to and including the current month for reimbursement.

Is there a minimum or maximum number of months I can submit for reimbursement at one time?

You can submit a minimum of one full month up to a maximum of 13 full months; however, the subsidy is only for expenses you have already paid.
